All fundraiser activity is mandatory or requires a buy-out contribution.

 

Tuition Assistance Process:

  1. Complete all required spaces on the form.
  2. Attach required income tax forms and W-2 forms.
  3. Answer all questions.
  4. Sign the form and date it.
  5. Enclosed the $16.50 fee (checks payable to Holy Name School).
  6. Return the form and all documents in the
    envelope to the school. (Please contact the school for application deadlines.)
